Before seeing Dr. Scott, you will need to fill out and submit an intake form that gathers detailed information about your health history. If you have any recent lab work, it should be included with this form. Before your visit, Dr. Scott will use this information to assess your health and begin putting together an individualized plan for you.
In your first visit, Dr. Scott will meet with you to better understand your individual health needs. A preliminary plan will be drawn up, and often you will be required to do “health homework”, which may involve completing further questionnaires, taking supplements, making diet or lifestyle changes, getting lab work done and or reading suggested material on your particular health challenge.
Often several visits will be required to resolve your health issues, and it is important to expect to make a minimum of 3-5 visits with Dr. Scott, ND per health concern.
Before seeing Dr. Scott, you will need to fill out and submit a short follow up form that helps Dr. Scott track your progress.
In your follow-up visits Dr. Scott will assess your progress, make necessary adjustments to treatments, suggest additional lab work or treatment, continue educating you on your particular health challenge, and answer any health related questions you may have.
